Why AI Scribe Is Revolutionizing Healthcare Documentation

How AI scribes draft visit notes from a recorded conversation, what they should and should not do, and how clinicians stay in control of the record.

Documentation takes up a large share of a clinician's day. A 2016 time-and-motion study by Sinsky and colleagues in the Annals of Internal Medicine found that ambulatory physicians spent nearly half of their office day on EHR and desk work, and many finished charts after hours. That after-hours "pajama time" is a familiar contributor to burnout.

Notes written late in the evening, from memory, are also harder to get right. And typing during the visit pulls attention away from the patient, which matters in every specialty and especially in psychiatry, where the conversation is the assessment.

An AI scribe records the visit, transcribes it and drafts a note from the conversation. The clinician then reviews, edits and saves it. Done well, this changes the clinician's job from writing the note to checking it.

Jogaza's AI scribe is built for psychiatric and behavioral health practices. Before recording starts, the clinician confirms the patient agreed to be recorded, and the chart shows whether an AI-consent form is on file. The scribe can record an in-person visit or run alongside a telehealth call, and it drafts the note for the visit type, such as a psychiatric initial evaluation or a medication follow-up, in the clinician's own voice.

Just as important is what it won't do. It never assigns a suicide or violence risk level and never invents a medication dose. Anything that wasn't discussed is marked as not discussed rather than filled in. The clinician reviews and edits every draft before saving.

Recordings of mental health visits are sensitive, so retention matters. In Jogaza, audio is deleted once it has been transcribed. A practice can also choose to have the full transcript removed when the note is signed, optionally keeping a short excerpt. Data is encrypted in transit and at rest, and every chart view and note change is recorded in an access log.

If you are evaluating any AI scribe, ask the vendor how patient consent is captured, what happens to the audio and transcript and when they are deleted, whether the draft ever fills in details that weren't said, and whether the clinician can edit every part of the note before it is saved.

An AI scribe doesn't replace clinical judgment. It takes the first pass at the note so the clinician can pay attention to the patient, and the clinician still owns what goes into the record. If that sounds useful for your practice, you can try Jogaza free for 14 days, with no card needed.
